CHA Health Forms Required for Campers

Immunization Form - required by all campers* 

Sunscreen and Insect Repellent - only required by BUCA & Schools Out!; it is included in the CHA Camp Registration Form for everyone else. 

Asthma Health Care Plan is required, along with the Administration of Medication Form if your child has asthma.

Allergy Health Care Plan is required, along with the Administration of Medication Form if your child has allergies.

Administration of Medication Form - required if your child has asthma or allergies that require medication. One form per medication please.

Individual Health Plan and an Administration of Medication Form - required if your child has a medical condition or illness (other than asthma or allergies) that requires medication. One form per medication please.

Suspected Allergy is for  children with a suspected food allergy or intolerance, but who do not have a medical diagnosis. If a diagnosis exists, a provider‑completed Health Care Plan is required.

*Already on file for enrolled CHA students.

Weekly: Summer fun for the whole week! When registering, select the week-long option which includes multi-age themed activities that vary each week. The weekly option is best for recurring campers and those wanting the full summer camp experience at Chestnut Hill Academy.

Daily: This option is for families wanting individual camp days.

Incoming Kindergarten Week: This is a week-long camp for our incoming CHA Kindergarten students. This camp will be offered during Week 7 (August 3rd-7th). Learn more here.

Back-Up Care (BUCA) and Microsoft School’s Out! benefits can be applied to all camp registration options.  

Additional Weeks for Families not using Benefits - If you have already registered your child(ren) for camp but would like to add extra weeks of Camp or Extended Day to your registration, please complete the Camp Explorations Additional Weeks order form. If you want to add additional lunch orders, please submit the Camp Explorations Lunch Order Form again, with only the additional lunches selected.

Using School’s Out! Benefit (Microsoft families) 

• Please log in to the Bright Horizons back-up care website here to register and make reservations. To use the Schools Out! Program, please select ‘Make a Reservation’ then select ‘Special Program’ on step 1 to display the Schools Out! Chestnut Hill Academy option. You can also go to ‘My Benefits’, then the Special Programs section, and select ‘Make a reservation’ from the Chestnut Hill Academy option there. If you have any issues registering or making a reservation, please call 1-877-BH-CARES (1-877-242-2737) for assistance. 
• No fees are collected directly by the school for camp, lunch, or childcare.
• Lunch is provided free of charge, but please use the Camp Explorations Lunch Order Form to select the lunches your child requires before they arrive for camp and choose the No Payment Required option.

Using Back-Up Care Benefit 

• To use camp through your employer’s Back-Up Care benefit, please log into the Bright Horizons Back-Up Care website here to register and make reservations. Depending on your benefit plan, there may be restrictions as to how far in advance you can register. When making a reservation, select the "Care For My Family" option, and select Chestnut Hill Academy as your first choice for care. If you have any issues registering or making a reservation, please call 1-877-BH-CARES (1-877-242-2737) for assistance.
• Lunch costs $8.50 per meal. Please use the Camp Explorations Lunch Order Form to order lunches before your child arrives for camp. Payment options are explained on the form. 
• No other fees are collected directly by the school for registration, camp, or childcare.

PLACEHOLDER FEE

• If you are planning to use your Back‑Up Care Advantage benefits and are registering on our website before your employer‑provided hours become available, please be aware of the following:

• Registrations completed directly through our website—prior to BUCA hours being accessible—will require a $100 placeholder fee.
  This fee and registration hold your spot until you are able to cancel and re‑register through your employee benefits.
• The $100 placeholder fee is non‑refundable.
  If you prefer to avoid this charge, we recommend waiting to register until your Back‑Up Care Advantage hours are available through your employer's benefits portal.

Current / Incoming CHA Family Not Using Benefits 

• Current families should register using the Childcare button in Blackbaud Resources
• If you are an incoming CHA family, please use the Non-CHA Family Camp Registration Form. If you have not submitted your enrollment forms yet, you will need to either complete your enrollment forms before your child starts camp or complete all the forms in the ‘CHA Health Forms Required for Campers’ section at the bottom of this page. Immunization records are mandatory for all campers; others depend on your child’s circumstances.
• The Registration Fee and Weekly Camp Fees will be billed in arrears via ACH.
• Lunch costs $8.50 per meal. Please use the Camp Explorations Lunch Order Form to order lunches before your child arrives for camp. Payment options are explained on the form. 

Non-CHA Family Not Using Benefits

• Please complete the Non-CHA Family Camp Registration Form and pay the Registration, Weekly Camp, and Extended Day fees listed on the form. Please keep the confirmation email you receive, as you will need the link it contains if you wish to make alterations to your registration.
• Please complete the forms in the ‘CHA Health Forms Required for Campers’ section at the bottom of this page before your child attends camp. Immunization records are mandatory for all campers; others depend on your child’s circumstances.
• Lunch costs $8.50 per meal. Please use the Camp Explorations Lunch Order Form to order lunches before your child arrives for camp. Payment options are explained on the form. 

Camp Lunches

Our cafeteria makes delicious, well-balanced lunches for $8.50 per meal. Lunches must be ordered in advance using the order form in the registration section above. Payment options are explained on the form. If you’re using Microsoft School’s Out! benefits, there is no charge, but we still need you to place an order.

Please note that CHA is a Nut Safe School and this policy applies during the summer. Products containing nuts will be disposed of and an alternative may be provided. If campers are bringing a home lunch, it should be a “ready-to-eat” item that does not require any additional preparation or warming. 

Camp is open Monday through Friday, from 9:00 to 4:00, and we recommend allowing time in the morning for drop-off routines. If you sign up for our Extended Day package or are using School’s Out and BUCA benefits, you can drop off at 7:30 and pick up at 5:30. After 5:30 a late pick-up fee of $10/minute will be incurred.

2:30-4:00 PM Theme-based camp activities and staggered recess schedules

4:00 PM Camp day ends! 

4:00-5:30 PM Afternoon Extended Day: snack, self-guided exploration and independent activities 

Your camper will have a personal locker in which they can store the belongings they bring each day.

• A backpack or bag is recommended along with a labeled water bottle.
• We highly recommend you send your child with two snacks. 
• We advise sending your camper with a rain jacket or extra layer each day. Younger students will want to bring a spare change of clothes in case of accidents.

• Parents/guardians must provide sunscreen for use during the camp day. Please make sure it is in the original bottle and the permission slip for that sunscreen is completed.

• Extra sun protection like a hat and sunglasses are optional. Closed-toe shoes are recommended most days, but sandals are acceptable if they have a backstrap. No flip flops, please.

EXTENDED DAY AND CHILDCARE FAQ’S

Do I have to register through both the Camp Registration form and the School’s Out system if I need care during conference days? If you are a Microsoft family utilizing the School’s Out or Back-Up Care benefit, then you must register through the School’s Out or Back-Up Care system.  If you are not a Microsoft employee, you must register through the Camp Registration Form above, which will be sent out in Campus Updates.

How do I sign up for the after-school care program, and how do I make payments for it? If you want your student to be enrolled in our after-school care program (Extended Day), you should indicate this on your TADS Registration. Payment for Extended Day will be drafted automatically via ACH.

Once I’m enrolled, can I add more weeks? Yes! More camp weeks mean more fun! You can add more weeks at any time, based on availability.  Additions can be made by registering through our Addition Weeks form, which can be found on our website.

Once I’m enrolled, can I make changes? Yes, if you registered through CHA, please email us at camp@chestnuthillacademy.com. If you registered through your employer's Back-Up care system, please reach out to them directly.

Can my child be grouped with their friends? Yes! We do our best to accommodate at least one friend request per camper. Please reach out to us by email prior to your first day of camp. Friend requests cannot be guaranteed, but we do our best. Please note that your camper may not be in the same pod as the week prior.

How are campers grouped for the week? Campers will be divided into Pods depending on their grade/age.

What should my camper bring to camp? A labeled water bottle, snacks, lunch (if not ordered in advance), and weather-appropriate clothing.
